Social Support Groups

What is Social Support?

It is a chance for older people (65 or over or 50 and over for Aboriginal or Torres Strait Islander people), with various abilities and to their unpaid carers to get out and be with other older people living in the community.

It promotes participation in social activities with others. It includes activities at centres and outings that help people to stay independent.

Using the ‘Wellness & Reablement’ approach you are encouraged to undertake activities of interest to you and we support you to be independent and connected within your community.

There are a wide range of social activities on offer which are in the form of group outings and centre-based groups at Clunes and Trentham.

What social activities are offered?

Social Activities & Group Outings

We use your ideas and feedback to plan activities which leave from Clunes, Creswick, Daylesford and Trentham.

There are many different things to do including:

• Lunches
• Shopping
• Shows
• Excursions
• Holidays
• Exercise programs
• Walk and Talk Groups

Centre Based Groups (these are also known as Planned Activity Groups)

These are based at Clunes and Trentham Community Health Centres on Tuesdays and/or Fridays from 10am to 3pm.

Groups meet in a friendly, relaxed and comfortable place. Transport to and from your home is available.

Accessing the program

The Program is a subsidised aged care service by the Australian Government Department of Health under the Commonwealth Home Support Program (CHSP) for older frail people over the age of 65.

To find out if you are eligible contact My Aged Care on 1800 200 422 and ask for an assessment.

Fees apply and the cost is dependent upon your eligibility for a subsidy. People in Residential Respite and those with a Home Care Package can also access the Social Support Program. The Coordinator will talk with you about any fees you may be required to pay and what is included in the cost.
